    Consent forms for new clients should be completed before arriving to your appointment to ensure we can get started right away!

    All clients must have a consent form on file. You may complete the form below.

  • Scheduling/ Late Policy

    Please give your best effort to be on time for your appointment! However, we understand things happen.

    Therefore, clients are able to cancel appointments up to 24 hours prior to the scheduled appointment without penalty. All appointments must be modified online.

    All late cancellations less than 24 hours will incur a 50% service fee.

    All clients are extended an 8 minute grace period as a courtesy (3 minute grace period for appointments less than 30 minutes).

    After the grace period, clients are considered no shows & the appointment is cancelled.

    All no shows are charged 100% of the service.

  • Payments

    All clients must have a working card on file that is preauthorized the day of the appointment. We understand some clients keep their cards locked, however these cards must be unlocked on the day of appointment. If we are unable to preauthorize the card on file, unfortunately we will have to cancel the appointment. This card will be charged at the time of service, or you may choose to pay with a different card or cash.

  • Communication

    All changes to appointments must be made online. Please do not text or dm to cancel/edit your appointment. Please text or email questions/concerns during business hours. Messages after hours will be responded to the following business day.
